1. ​Click the Sign in button and enter your User Name and Password (or go to Step 2).

2. Forgot your password? Click on “Find your User name/Reset your Password” at the bottom of the page. It will open a new window.

3. Enter your 9-digit OSIS number.

4. Enter your date of birth.

5. Click Continue and save your password in a safe place.
